Care Assessor/ Coordinator

We are seeking an enthusiastic and motivated Care Assessor/ Coordinator to join our Maldon based, family run Care and Nursing Agency.

This full time position in our established team, includes 4 week days and every other weekend working from home e.g. Friday pm-Sunday am or Sunday am- Monday am.

Core duties and responsibilities

• Experience within Health and social care, including completing Risk Assessments, Mental Capacity Assessments and care planning.

• Attend care assessments and reviews in the community using your own vehicle.

• Computer literate with the ability to adapt to new systems.

• Team player with strong interpersonal skills with the ability to build a rapport quickly.

• Have experience working with other Health and Social care professionals for the benefit of the Service User.

• Ensure phone and emails are answered promptly and people are communicated with in a polite and respectful manner. • Use our electronic care system to complete in depth, person-centered care plans.

• Roster care packages and respond to ad-hoc care requests, using an electronic system.

• Add and maintain Service User information onto the electronic system.

• Flexibility to cover the paid-on call every other weekend and to meet the needs of the business. • Must hold a full UK driving license. Use of own reliable vehicle required.

Essential

• Full UK driving license and access to own vehicle.

• Clear enhanced DBS.

• Experience in Health and Social Care or a similar role.

• CV required, including full employment history.

• Excellent communication and organisational skills.
